Beginning of School Year Forms

In our attempt to Go Green, the district has made a conscience effort to reduce the amount of paper sent home each year at the start of school. In line with this shift we made the necessary and required district documents accessible via our website. Volusia County Schools would like to assist parents with accessing the forms required for you child(ren) at the beginning of the school your and has accomplished this by making the documents available on-demand from any internet connected device.

Income Based Services Request

Ensuring that our students have access to every opportunity is a priority for our district. The purpose of the Income Based Services Request is to determine eligibility for income based programs and benefits.  If your family wishes to be considered for services that are based on income, please complete this form and submit it to your school.  If you have a direct certification letter that was mailed to your home, you do not need to complete this form.  The letter can serve as income verification. This information will be considered confidential and will only be used to determine program eligibility.

1:1 Device Acknowledgement Form

Technology access has been established for educational purposes and will be consistent with the district's curriculum and the State of Florida Standards. Student in grades 6-12 have access to a device through the 1:1 program.

Device Insurance Information

In light of Hurricane Ian, Volusia County Schools has graciously extended the deadline for middle school and high school families to sign up for the optional technology device insurance until Friday, October 14, 2022. The cost is $30 per device. However, it’s free for families who qualify for income-based services such as Free and Reduced Lunch, but signing up with MySchoolBucks is still necessary, the price will just be $0. This insurance reduces the cost of mild to severe damage and replacement cost of a lost or stolen device. The insurance does not cover a lost laptop sleeve (middle school only) or lost charger.
